Credit Reporting Services Subscriber Agreement		Informative Research's Credit Reporting Services Subscriber Agreement - choose one of the three agreements below based on your business type:

Credit Card Payment Authorization Form (Word)
	(Word)	Use this authorization form to provide a credit card for your business' account with Informative Research.
	(Word)	Use this authorization form if you are a Loan Officer or Agent providing your credit card for individual billing under your firm's established Informative Research account.
Assetwise Direct^SM		This agreement applies to lenders or bankers that wish to order credit from the RFC Assetwise Direct system.

Home-Based Business Addendum		This addendum is required for all Informative Research customers who operate from a residential location.

Credit ReScore Addendum		This addendum applies to bankers, lenders, and brokers wanting to use our Credit ReScore product. If using Credit ReScore services only, you must complete the subscriber agreement in addition to this addendum.

Further Agreement		Does your business have multiple owners with equal or nearly equal stake in the company? Each of them will need to sign IR’s Further Agreement. Those not signing the Credit Services Subscriber Agreement itself should use this standalone Further Agreement form.

Credit ReScore Request Form		Credit ReScore Request Form - This form provides the necessary information to initiate a bureau rescore (requestor account must be authorized for Credit ReScore). If you are not authorized for Credit ReScore, please contact your Sales Account Executive.

Borrower Consent Form		An authorization form from the borrower that allows Informative Research to research and validate tradeline and public record accuracy.

Medical Authorization Form		Medical Authorization Form - This form allows a borrower to give authorization to Informative Research to access and investigate privately coded medical information.

3rd Party Credit Card Authorization		Are your business expenses charged to another individual’s credit card (e.g., the owner of your company)? Have him or her complete this Third-Party Credit Card Authorization Form. (Please note that this credit card will be charged for all future transactions on your account. This form is not to be used with a borrower’s credit card.)
